Job summary

Humana is seeking a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care, conducting in-home assessments, developing care plans, and coordinating with providers and families. This field role requires extensive local travel to visit members and ensure services meet needs.

Responsibilities include coordinating care, addressing barriers, and guiding enrollee to resources while maintaining HIPAA compliance and documenting outcomes.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Unrestricted Licensed Practical Nurse (LPN) in Illinois with one year experience.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and Long-Term Care Facilities with 75%-90% local travel.
• Assess member care needs and ensure services in least restrictive setting.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care plan against personal goals.
• Monitor patient progress and adjust care planning as needed.
• Assess enrollee needs via telephonic and in-person evaluations to coordinate services.
• Assist enrollee with addressing gaps in care and accessing services.
• Assist enrollee with filing grievances and appeals.
• Provide information about MCO requirements, services, and benefits.
• Refer enrollees to community resources and guide families to appropriate care resources.
