Job summary

Humana Inc. seeks a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care. You will outreaching to newly enrolled members, perform HRAs, identify immediate needs, and connect to services to establish a foundation for ongoing care.

Role involves 75% travel to members’ homes or care facilities, coordinating services, and guiding families toward resources while addressing barriers to care.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• LPN license in Illinois with 1 year of experience conducting assessments.
• Licensed Professional Counselor (LPC) license.
• Licensed Social Worker (LSW) license.
• Must reside in Illinois; 1+ year in health care/case management.
• Experience with Medicare/Medicaid recipients and/or long-term care.

Responsibilities

• Visit Medicaid enrollees in homes, facilities or other care settings with 75% local travel.
• Assess care needs, plan interventions, and coordinate services with care teams.
• Conduct Health Risk Assessments and track enrollee progress toward goals.
• Assist with grievances and appeals and provide program information.
• Guide enrollees to community resources and ensure access to services.

Education

Bachelor's degree in social sciences, social work, human services, or a related field
