Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and care coordination activities. You will work with a multidisciplinary team to deliver integrated member care across the continuum, aiming for desired outcomes and cost-effective services.

The role involves conducting assessments, developing care plans with members and providers, and performing follow-ups to ensure progress and adherence to plan. Some local travel (25-40%) may be required.

Qualifications

• 2+ years in health care, preferably care management or related setting.
• Knowledge of community resources.
• Proactive, detail-oriented, and able to work with diverse populations.

Responsibilities

• Complete member assessments within regulated timelines to identify care coordination eligibility.
• Develop and implement care plans with member, family, and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans, document interventions, and adjust as needed.
• Maintain ongoing member caseload with regular outreach.
• Coordinate services across behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and collaborate with the care team.
