Job summary

Molina Healthcare is seeking a field-based Care Coordinator to support care management and care coordination activities. You will collaborate with a multidisciplinary team to deliver integrated member care across the continuum, aiming for quality outcomes and cost-effective services.

The role requires conducting assessments, developing care plans, and coordinating services with members, families, and health professionals.

Qualifications

• At least 2 years health care experience, preferably in behavioral health, or equivalent education/experience.
• Licensed behavioral health clinician license active and unrestricted in state of practice.
• Valid driver's license, reliable transportation, and auto insurance for travel requirements.

Responsibilities

• Completes comprehensive behavioral health assessments per regulated timelines and identifies candidates for care coordination.
• Develops and implements care plans with members, caregivers and health professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing and Molina clinical guidelines to support change.
• Assesses barriers to care and provides coordination assistance.
• Provides consultation and resources to peers as needed.
• Travel 25-40% may be required based on state/contractual requirements.
