Job summary

Molina Healthcare seeks a Care Coordinator to support care management and coordinate services across the continuum in Illinois. You will collaborate with a multidisciplinary team to ensure member progress toward outcomes and contribute to a strategy of quality and cost-effective care.

The role involves assessments, care plan development, and both telephonic and in-person visits, with travel of 25-40% as required by state contracts. Strong communication and organizational skills are essential.

Qualifications

• At least 2 years experience in health care or care management.
• Clinical licensure and/or certification as required by state contract.
• Valid driver's license and ability to travel locally as needed.
• Knowledge of community resources and ability to work with diverse populations.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes assessments within regulated timelines to identify care coordination eligibility.
• Develops and implements care plans with members and care networks.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health and LTSS.
• Facilitates interdisciplinary care team meetings and collaboration.
• Assesses barriers to care and assists members in addressing them.
• Travels 25-40% locally as required by state/contractual needs.
