Job summary

Qualifications

• At least 2 years experience in health care, preferably in care management.
• Valid driver’s license and reliable transportation for local travel.
• Knowledge of community resources and ability to work independently.
• Pro-active, detail-oriented with strong communication skills.

Responsibilities

• Complete member assessments within regulated timelines.
• Develop and implement care plans with member and care team.
• Conduct telephonic, in-person, or home visits.
• Monitor care plan effectiveness and adjust as needed.
• Maintain a local caseload and coordinate with ICT.
• Travel 25-40% as required.
