Job summary

Molina Healthcare is seeking a dedicated care coordinator to support behavioral health care management across the continuum. You will collaborate with a multidisciplinary team to ensure quality, cost-effective member care and progress toward outcomes.

You will conduct assessments, develop care plans, and coordinate services, including home visits when needed. Expect 25-40% local travel and strong communication with the care team.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Active and unrestricted license in state of practice.
• Valid driver's license with reliable transportation.
• Experience with clinical platforms and data entry.
• Excellent verbal and written communication skills.

Responsibilities

• Performs comprehensive behavioral health assessments per regulated timelines and determines eligibility for care coordination.
• Develops and implements care plans with member, caregiver, physician, and other health professionals.
• Conducts telephonic, face-to-face, or home visits as required.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and provides peer consultation as needed.
• Travel 25-40% as required.
