Job summary

Molina Healthcare is seeking a Care Management professional to support care coordination across the continuum. You will assess members, develop care plans with the team, and conduct visits as required to ensure progress toward outcomes.

The role emphasizes collaboration with multidisciplinary teams, proactive outreach, and effective communication. Travel up to 40% may be required based on state contracts.

Qualifications

• At least 2 years experience in health care, care management or related setting.
• Clinical licensure/certification required only if mandated by state contract/regulations.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.
• Demonstrated knowledge of community resources.
• Ability to work independently and manage multiple tasks.
• Strong verbal and written communication skills.

Responsibilities

• Completes assessments of members per timelines and determines eligibility for care coordination.
• Develops and implements care plans with member and care team to address needs and goals.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload and outreach for management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
