Job summary

Molina Healthcare is seeking a dedicated Care Coordinator to support care management and coordinate services across the continuum. You will work with multidisciplinary teams to ensure quality, cost-effective member care and to meet regulated timelines, with travel as required.

The role emphasizes assessment, care planning, and ongoing monitoring to improve outcomes, while collaborating with licensed managers and care professionals in a dynamic health care environment.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent.
• Valid driver's license, reliable transportation, and auto insurance for travel requirements.
• Demonstrated knowledge of community resources.
• Ability to work independently and manage multiple projects.
• Excellent communication and time-management skills.

Responsibilities

• Complete member assessments per regulated timelines and determine eligibility for care coordination.
• Develop and implement care plans with member and care team.
• Conduct telephonic, face-to-face or home visits.
• Monitor care plan effectiveness and document interventions.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration.
• Educate and motivate change using motivational interviewing and Molina guidelines.
• Address barriers to care and provide coordination assistance.
• Collaborate with licensed care managers as needed.
• Travel up to 25–40% depending on state/contractual requirements.
