Job summary

Sacramento Children's Home is seeking a Family Facilitator to perform assessment services and develop wraparound plans for youth and families in the Full Service Partnership Wraparound program. You will coordinate CFT meetings, ensure timely access to services, and collaborate on crisis management and service delivery.

The role requires knowledge of HFW, FSP, and crisis stabilization models, plus the ability to work with diverse families and communities.

Responsibilities

• Contribute to assessment, development, implementation, and evaluation of wraparound plans.
• Coordinate, schedule, and facilitate child and family team meetings (CFT).
• Provide oversight for timely CFT access and crisis meetings.
• Coordinate services for families based on CFT needs (psychiatric, therapy, referrals).
• Lead and implement service models and resource coordination.
• Monitor and update plans with LPHA feedback.
• Communicate symptom information to LPHA for correct diagnosis in EHR.
• Provide direct services and crisis management.
• Participate in Wrap crisis on-call system.
• Complete clinical bundles in EHR (Cal-AIMs, CANS, etc.).
• Write progress notes within deadlines.
• Prepare documents (CFT minutes, safety plan, etc.).
• Support ongoing development and evaluation of the service program.
• Assist in staff supervision and meetings.
