Sacramento Children's Home is recruiting a Family Specialist (Skills Trainer) to provide direct services to children and families under the FSPW program in Sacramento, CA. The role focuses on delivering concrete supports aligned with individualized service plans and wraps around families to achieve functional goals.

Responsibilities include community-based interventions, collaboration with schools and social services, and ongoing documentation.

Qualifications

• At least one year of full-time experience in the mental health field is required.
• Two years of college or an AA degree in Human Services or related fields preferred.
• Ability to read, write, speak English; able to work with diverse families.
• Must be at least 21 years old and have a valid driver's license.

Responsibilities

• Provide direct services to children and families per individualized service plans.
• Engage with families to address daily living tasks and rehabilitation goals.
• Collaborate with team and external providers to coordinate care.
• Maintain accurate documentation and participate in Wrap program activities.
