Family & Children's Services seeks a wraparound facilitator to coordinate child and family teams, advocate for families, and connect them with community resources. The role includes crisis planning, documentation, and maintaining confidentiality while coordinating with schools, DHS, and education systems.

The candidate will support families in achieving long-term stability through the wraparound process, empowering youth and caregivers to access services and optimize outcomes.

Qualifications

• Bachelor's degree is required for this role.
• Must meet ODMHSAS and HOPE requirements and maintain DMHSAS CM II certification.
• First Aid and CPR certifications may be required; valid Oklahoma driver’s license.
• Ability to read, write and document clinical information accurately.

Responsabilités

• Facilitates wraparound/service coordination with families and youth.
• Advocates for families and links to community resources.
• Maintains confidentiality and documents services per policy.
• Develops and updates crisis plans and recovery goals with the family.
• Provides transportation support using personal vehicle or agency vehicle.
