Counseling and Research Associates, Inc. is seeking a Full Service Partnership - High Fidelity Wraparound (FSP-HFW) Facilitator.

• Facilitates Child and Family Team (CFT) meetings.
• Engages child/youth and family/caregiver in planning and decision-making.
• Maintains safety and crisis protocols as required.

Responsabilités

• Identify and engage CFT members and lead the planning of Plan of Care.
• Develop and implement individualized Plans of Care with defined goals and timelines.
• Coordinate with schools, healthcare providers, and community partners to ensure continuity of care.
• Monitor plan effectiveness and adjust goals as needed.
• Provide crisis planning and post-crisis follow-up as part of the care team.
