Job summary

Sccs4kids in California is seeking a Wraparound Facilitator to implement the High-Fidelity Wraparound (HFW) process with fidelity. You will coordinate the Child and Family Team, develop safety plans, and tailor the Plan of Care to each family’s strengths and needs.

You will lead team meetings, engage natural supports, and support transitions toward independence, while maintaining a strengths-based, culturally responsive approach for families facing behavioral health challenges.

Qualifications

• Bachelor’s Degree in social work, psychology, nursing, occupational therapy, or related field preferred.
• One year experience in human services, preferably case management.
• Must qualify as a Medi-Cal Specialty Mental Health Services (SMHS) provider. Eligible activities may be claimed under Medi-Cal if all state and federal Medicaid requirements are met.
• Completion of Statewide Standardized CFT Facilitation Training via RCFFP/CACWT.

Responsibilities

• Actively participate in engagement with families and support the Wraparound process.
• Schedule, convene, and facilitate Wraparound team meetings with natural and formal supports.
• Lead development of Wraparound team agreements, family vision, and measurable outcomes in the Plan of Care.
• Coordinate crisis and safety planning reflecting family-driven strategies.
• Maintain ongoing communication with all team members to ensure service delivery.
