Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing treatment plans, delivering therapeutic interventions, and linking them to needed services. This is a community-based role operating in clients' homes with emphasis on regulatory compliance and thorough documentation.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources as identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring needs are met.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes in compliance with policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development activities.

Education

Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field
