Job summary

HEALTH CONNECT AMERICA, INC is seeking a Family Development Specialist to support clients and families by developing treatment plans and providing therapeutic interventions. This role includes effective communication and compliance with regulatory standards while actively recruiting new referrals.

Qualifications

• Bachelor’s degree in a human services discipline such as Social Work, Psychology, or Counseling.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients.
• Provide therapeutic interventions and skill-building.
• Link clients and families with specific services.
• Maintain accurate documentation of client interactions.
• Provide 24/7 on-call support for clients when required.
