The position requires an associate degree or equivalent, with bilingual preferred and 1–2 years in a community-based setting. Strong MS Office skills, writing, and teamwork are essential for meeting grant outcomes and program goals.

Qualifications

• Required associate degree or equivalent based on relevant work experienced required.
• Bilingual or multi-lingual preferred (written and oral proficiency).
• Preferred: prior experience in multi-cultural setting and/or community health worker certification.
• Ideal candidate will have a bachelor’s degree with knowledge in social work, human development, or related field.
• Experience in community-based settings for 1–2 years preferred.
• Proficient in MS Office products (Word, Excel, PowerPoint, Teams, Outlook).
• Excellent writing and interpersonal skills; ability to work in a team.

Responsibilities

• Help clients and families navigate and access community services.
• Promote, maintain, and improve holistic health (physical, emotional, spiritual).
• Provide social support, informal counseling, and advocacy for clients with providers.
