Responsibilities

• Conduct outreach and recruitment for Family Resource Center activities.
• Maintain professional relationships with families through regular contact.
• Administer intake, assessments and screening tools.
• Set goals and activities based on intake/assessment results.
• Provide intensive case management to families on caseload.
• Coordinate services, resources, transportation, and basic needs support.
• Record services and activities in PEIRS and Salesforce.
