Job summary

BCFS Health and Human Services seeks a Resource Navigator to recruit participants, build trusting relationships with families, and manage intake, assessments, and service planning. You will coordinate resources, document activities, and conduct follow-ups to ensure program goals are met.

Qualifications

• High school diploma required; Bachelor’s preferred.
• 3+ years of related work experience with HS diploma; 1+ year with Bachelor’s degree preferred.
• Must be authorized to work in the U.S. without sponsorship.

Responsibilities

• Outreach and recruitment of participants for Family Resource Center activities.
• Establish professional relationships with families and provide regular contact.
• Administer intake, assessments, and screening tools.
• Use results to set goals and plan activities with parents.
• Provide intensive case management and coordinated services to families.
• Record services and participate in follow-ups with families and supervisor.
