BCFS is seeking a Resource Navigator to support families through outreach, intake, and comprehensive case management services. You will partner with families to identify needs, coordinate community resources, and track progress within our approved program systems.

The role requires strong communication, cultural sensitivity, and the ability to work in a fast-paced environment. Travel may be required to serve families across the region and to participate in trainings and community events.

Formación

• Experience in social services or family support is desirable.
• Ability to document and track activities in case management systems.

Responsabilidades

• Conduct outreach and recruitment for Family Resource Center activities.
• Establish professional relationships with families and provide regular contact.
• Perform intake, assessments, and screenings with clients and family members.
• Use results to set goals and plan activities with parents.
• Provide intensive case management and coordinate services and referrals.
• Assist with basic needs, transportation, and other supports for families on caseload.
• Conduct systematic follow-ups for families and document all interactions.
• Collaborate with team to run family activities and caregiver groups.
• Record services in PEIRS (DFPS) and Salesforce (BCFS) accurately and timely.
• Attend trainings and meet deliverables while following grant requirements.
