Job summary

Qualifications

• High School diploma or equivalent required; an Associate degree preferred.
• Valid Ohio driver’s license and proof of auto insurance; driving background check required.
• CHW certification by Ohio Board of Nursing or completion of CHW training; certification within 12 months.
• Strong customer service, communication, and interpersonal skills; ability to work in diverse teams.
• One year of professional social service or health-related experience; experience with children/families preferred.
• Proficient with Word and Excel; ability to travel and work flexible hours.

Responsibilities

• Link patients and families to essential support services with clinical staff.
• Address SDH needs and improve access to community resources.
• Develop and implement health education programs and materials.
• Collect and report data on program activities.
• Maintain relationships with housing resources, food pantries, utilities, agencies.
• Follow up with patients to confirm resource linkage and assess barriers.
• May accompany patients to social service appointments.
• Document interactions in EPIC.
• Stay informed on community health trends and prevention efforts.
