Job summary

Nationwide Children's Hospital is seeking a Community Health Worker at our Main Campus in Columbus, OH. You will collaborate with clinical staff to connect patients and families with social determinants of health resources and provide health education.

The role requires a high school diploma, Ohio CHW certification (or completion within one year), a valid driver's license, and the ability to travel. Associates degree preferred; one year of relevant experience is expected.

Qualifications

• High School Graduate or equivalent, required.
• Associates Degree, preferred.
• CHW certification by Ohio Board of Nursing or completion of CHW Training Program; certification within one year.

Responsibilities

• Links patient and family to essential support services in collaboration with clinical staff.
• Develops and implements health education programs and materials; collects and reports data.
• Establishes relationships with housing resources, food pantries, utility companies, government agencies, and community orgs.
• Assists in follow-up calls to confirm linkage and to assess barriers; may accompany to social service appointments.
• Documents patient and family interactions in EPIC; stays informed on trends and prevention efforts.
