Nationwide Children’s Hospital in Columbus seeks a Community Health Worker to collaborate with clinical staff, providing culturally sensitive one-to-one support to patients and families with social determinants of health needs.
Candidates must reside in Ross, Pike, or Scioto counties and have a high school diploma (Associates degree preferred); valid Ohio driver's license; CHW certification within one year; ability to travel and work flexible hours including some evenings/weekends.
MUST reside in Ross, Pike, or Scioto county.
Collaborates with clinical and medical staff to provide culturally sensitive one-on-one support and guidance to patients and families with social determinants of health (SDH) needs.
High School Graduate or equivalent, required. Associates Degree, preferred.
Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
Community Health Worker certification by the Ohio Board of Nursing required or Completion of the Community Health Worker Training Program; certification must be obtained within one year of employment.
Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers. Ability to work as a part of a team, with diverse populations, (i.e. racial/ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings. Strong attention to detail and organizational abilities required. Sound decision making skills, with the ability to maintain highly confidential information. Moderate computer skills required (familiarity with Microsoft Word and Excel, preferable). Ability to travel and work flexible hours, including some evenings and weekends.
One year of professional experience with a social service agency, community-based program, or health-related institution, required. Experience working or volunteering with children or families, required.
