Job summary

Qualifications

Responsibilities

• Assist families in determining needed resources and services through screening and triage.
• Answer questions and provide service referrals and scheduling.
• Gather referral and pre-admission information and complete pre-admission data entry.
• Identify emergent or complex cases and seek clinical review prior to finalizing referrals.
• Collaborate with interdisciplinary teams to ensure efficient resource and scheduling services.
• Use Electronic Medical Record and Scheduling software to assess demand and service capacity.
• Maintain confidentiality of all clinical and administrative information per policy.
