Job summary

Candidates must reside in Ross, Pike, or Scioto counties and have a high school diploma (Associates degree preferred); valid Ohio driver's license; CHW certification within one year; ability to travel and work flexible hours including some evenings/weekends.

Qualifications

  • High School Diploma or equivalent required.
  • Associates Degree preferred.
  • Valid Ohio driver’s license and proof of auto insurance required.
  • CHW certification within one year of employment.
  • Pass motor vehicle background inspection and driving qualifications.

Responsibilities

  • Links patient and family to essential support services with clinical staff.
  • Addresses SDH needs and improves access to community resources.
  • Develops and implements health education programs and materials.
  • Collects and reports data on program activities.
  • Maintains relationships with housing resources, food pantries, utilities, agencies.
  • Assist in follow-up calls to confirm linkage and navigate resources.
  • May accompany patients to social service appointments.
  • Documents interactions in EPIC and stays informed on community health trends.
