Job summary

Qualifications

• High School diploma or equivalent is required.
• Associate degree is preferred.
• Valid Ohio driver’s license is required.
• Community Health Worker certification or completion within one year of employment.

Responsibilities

• Links patients and families to essential support services identified by clinical staff.
• Develops health education programs and materials.
• Maintains relationships with housing resources, food pantries, and community organizations.
• Conducts follow-up calls to confirm linkage and assess barriers.
• Documents interactions in EPIC and maintains confidentiality.
