Job summary

Qualifications

Responsibilities

• Links patients to housing, food, utilities and services in collaboration with clinical staff.
• Develops health education programs and reports data on activities.
• Maintains relationships with housing, food pantries, utilities and agencies.
• Performs follow-up calls to confirm linkage and help navigate resources; may accompany to appointments.
• Documents patient interactions in EPIC accurately.
• Keeps informed on trends and prevention efforts for the community.
