Job summary

Talentify is seeking a Community Health Worker to collaborate with clinical staff in supporting patients and families with social determinants of health needs.

You will link clients to housing, food resources, utilities, and other community services, while delivering health education and collecting program data. A valid OH driver's license and CHW certification within a year are required.

Qualifications

  • High School Graduate or equivalent, required.
  • Associates Degree, preferred.
  • Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
  • Community Health Worker certification by the Ohio Board of Nursing required or Completion of the Community Health Worker Training Program; certification must be obtained within one year of employment.
  • One year of professional experience with a social service agency, community-based program, or health-related institution, required.
  • Experience working or volunteering with children or families, required.

Responsibilities

  • Remains informed of trends, developments, as well as state-of-the-art prevention efforts and strategies, and general information regarding the targeted community.

  • Ability to work as a part of a team, with diverse populations, (i.e. racial/ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.
  • Strong attention to detail and organizational abilities required. Sound decision making skills, with the ability to maintain highly confidential information.
  • Moderate computer skills required (familiarity with Microsoft Word and Excel, preferable).
  • Ability to travel and work flexible hours, including some evenings and weekends.
Experience:
  • One year of professional experience with a social service agency, community-based program, or health-related institution, required.
  • Experience working or volunteering with children or families, required.
