Health Connect America, Inc. seeks a Family Development Specialist to deliver home- and community-based therapies, develop plans, and connect families with needed services. You will collaborate with the treatment team to tailor care and drive positive outcomes.

You will recruit referrals, document progress, and participate in weekly team meetings while ensuring regulatory compliance and high-quality service delivery for children, families, and adults.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsabilités

• Develop individualized treatment plans and coordinate care with the treatment team.
• Provide individual, group, and family therapeutic interventions.
• Attend weekly Treatment Team meetings.
• Link clients and families with specific services and resources.
• Serve as a liaison with professionals, agencies, and community resources.
• Maintain accurate documentation of all client interactions and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development activities.
