Job summary

Health Connect America is hiring a Family Development Specialist to support clients and families by creating and implementing treatment plans and delivering therapeutic interventions in home, community, or office settings in Tennessee. You will coordinate with the treatment team, ensure thorough documentation, and actively recruit referrals to sustain a full caseload.

Qualifications

• Bachelor's degree in a human services field required.
• Minimum of two years' experience with children and/or families.

Responsibilities

• Develop individualized treatment plans with input from the treatment team.
• Provide individual, group, and family therapeutic interventions.
• Attend and participate in weekly Treatment Team meetings.
• Link clients to services identified in the treatment plan.
• Coordinate care with professionals, agencies, and community resources.
• Document client interactions and progress in compliance with standards.
• Recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
