Job summary

Health Connect America in Tennessee seeks a qualified Family Support Specialist to deliver best-practice clinical services for children, families, and adults. You will work across home, community, and office settings, develop treatment plans with the treatment team, and provide individual, group, and family therapies.

The role emphasizes documentation, multidisciplinary collaboration, on-call support, and outreach to recruit referrals to sustain caseloads.

Qualifications

• Bachelor's degree in a human services discipline or related field; OR BA/BS with 3 years of relevant experience.
• 1 year of experience working with children (including internships/volunteer) preferred.

Responsibilities

• Communicate with clients, families, and professionals to answer questions and address concerns.
• Develop treatment plans with input from the treatment team and supervisor.
• Provide individual, group, and family therapeutic interventions as outlined in the plan.
• Deliver evidence-informed therapeutic interventions with clients and families.
• Complete client documentation within established timeframes.
• Attend weekly Treatment Team meetings and coordinate services.
