Job summary

Barrette Workforce Solutions, LLC is seeking an Admissions Nurse (RN) for an evening shift (3:00 PM – 11:00 PM) at a Connecticut senior living and healthcare provider. You will oversee clinical intake for incoming residents and coordinate with hospital discharge planners, physicians, and families to ensure safe transitions into the center.

Responsibilities include initial nursing assessments, medication reconciliation, and setting up electronic charts, while guiding staff and residents through

Qualifications

• Active, unrestricted RN license in CT.
• Experience handling admissions or discharge coordination is highly desirable.
• Experience with EMR systems for rapid chart initiation.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM – 11:00 PM).
• Conduct initial nursing assessments and admission documentation.
• Reconcile hospital discharge summaries and orders to ensure medication continuity.
• Coordinate with discharge planners, physicians, and families for room setup and equipment.
• Develop preliminary care plans and notify on-duty supervisors of critical needs.
• Orient new residents and families to facility policies and routines.
