Job summary

Meritas Advisory Group, LLC seeks an Admissions Nurse (RN) to manage evening clinical intake for new residents in a Connecticut senior living setting. The role focuses on intake, assessment, medication reconciliation, and initial chart setup to support staff during the 3:00 PM–11:00 PM shift.

Ideal candidates hold an active CT RN license with 1–2 years in SNF/LTC or sub-acute care and experience with admissions, discharge coordination, and EMR systems such as PointClickCare.

Qualifications

• Active, unrestricted CT RN license.
• 1–2 years of nursing experience in SNF/LTC or sub-acute rehab.
• Experience with admissions and discharge coordination preferred.
• Experience with EMR systems (PointClickCare) for rapid chart initiation.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM).
• Perform initial nursing assessments and admission documentation.
• Reconcile hospital discharge summaries and physician orders for medication continuity.
• Coordinate with discharge planners, care teams, and families for room setups and equipment.
• Draft preliminary individualized care plans and communicate needs to on-duty supervisors.
• Orient new residents and families to facility policies and routines.
