Job summary

Red Seal Recruiting Ltd. is seeking an Admissions Nurse (RN) for an established Connecticut senior living and healthcare organization.

The role focuses on evening admissions (3:00 PM – 11:00 PM) and oversees clinical intake, medication reconciliation, and initial electronic chart setups to ensure a safe transition for new residents. You will lead admission documentation, perform initial assessments, coordinate with hospital discharge planners, physicians, and families, and help orient residents

Qualifications

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC or sub-acute rehab.
• Experience with admissions or discharge coordination.
• EMR experience with PointClickCare for rapid chart initiation.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM – 11:00 PM).
• Perform initial nursing assessments and admission documentation.
• Reconcile discharge summaries and orders for continuity of care.
• Coordinate with discharge planners, physicians, and families.
• Initiate care plans and communicate resident needs to on‑duty staff.
• Orient residents and families to facility policies and routines.
