Job summary

Confident Search Consultants is seeking an Admissions Nurse (RN) in Connecticut to oversee and streamline clinical intake for incoming residents. The role focuses on evening admissions (3:00 PM – 11:00 PM) and requires coordination with hospital discharge planners, physicians, leadership, and families to ensure safe transitions into the center.

Responsibilities include initial nursing assessments, medication reconciliation, and setup of electronic charts so floor staff can focus on direct care.

Qualifications

• Active, unrestricted RN licensure in Connecticut.
• 1–2 years nursing experience in SNF/LTC or sub-acute settings preferred.
• Experience with admissions coordination or discharge planning is highly desirable.
• Hands-on EMR experience, e.g., PointClickCare, for rapid chart initiation.
• Strong organizational and communication skills to engage families and hospital teams.

Responsibilities

• Lead evening clinical intake and onboarding for admissions 3–11 PM.
• Perform initial nursing assessments and comprehensive admission docs.
• Reconcile discharge summaries and orders to ensure medication continuity.
• Collaborate with discharge planners, care teams, and families on setup.
• Develop preliminary care plans and communicate needs to on-duty supervisors.
• Orient residents and families to policies, routines, and staff roles.
