The Recruiting Hive is seeking an Admissions Nurse (RN) in Connecticut for an evening shift (3:00 PM – 11:00 PM). This full-time, on-site role focuses on incoming resident assessments, medication reconciliation, and initial electronic chart setup to support seamless transitions from hospital to facility.

You will coordinate with discharge planners, physicians, and families to prepare rooms and equipment, develop preliminary care plans, and orient new residents to policies.

Qualifications

• Active RN license in Connecticut (unrestricted).
• 1–2 years of admissions or SNF/LTC experience preferred.
• Experience coordinating hospital discharge and admissions.

Responsabilités

• Lead clinical intake and onboarding procedures during the evening shift (3:00 PM – 11:00 PM).
• Conduct initial nursing assessments, skin evaluations, and admission documentation.
• Reconcile discharge summaries and orders to ensure medication continuity.
• Partner with discharge planners, care teams, and families to prepare rooms and equipment.
• Initiate preliminary care plans and communicate needs to on‑duty supervisors.
• Orient new residents and families to facility policies and routines.
