Job summary

Markent Personnel is seeking an Admissions Nurse (RN) in Connecticut for an on-site, full-time role with an evening shift (3:00 PM – 11:00 PM). You will oversee clinical intake, admissions, and medication reconciliation to ensure safe transitions for new residents.

Responsibilities include initial assessments, documentation, coordinating with hospital discharge planners and families, and setting up rooms and equipment. Strong communication and organizational skills are essential.

Qualifications

• Active, unrestricted CT RN license.
• 1–2 years in SNF/LTC/sub-acute rehab preferred.
• Admissions or discharge coordination experience desirable.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM).
• Perform initial nursing assessments and complete admission documentation.
• Reconcile hospital summaries, orders, and pharmacy profiles for continuity.
• Coordinate with discharge planners, care teams, and families for room setup.
• Initiate preliminary care plans and flag urgent needs to supervisors.
• Orient new residents and families to policies, routines and staff.
