Hire Consulting Services seeks an Admissions Nurse (RN) in Connecticut to oversee evening clinical intake and streamline transitions for new residents. You will coordinate with discharge planners, physicians, and families to ensure safe admissions and immediate chart setup.

Responsibilities include initial nursing assessments, medication reconciliation, and onboarding. Licensing must be current in CT, with 1–2 years in SNF/LTC preferred.

Competenze

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC/sub-acute rehab setting preferred.
• Prior admissions, discharge coordination, or unit management/desk nursing experience desirable.
• Hands-on EMR experience with PointClickCare or similar systems.

Mansioni

• Lead clinical intake and onboarding for evening admissions (3:00 PM – 11:00 PM).
• Conduct initial nursing assessments, skin evaluations, and admission documentation on resident arrival.
• Reconcile hospital discharge summaries with physician orders and pharmacy profiles to ensure medication continuity.
• Collaborate with hospital discharge planners, care teams, and families to arrange room setups and equipment.
• Develop preliminary individualized care plans and flag resident needs to on-duty supervisors.
• Orient new residents and families to facility policies and care routines to ease transition.
