Top Echelon Software, LLC is seeking an Admissions Nurse (RN) for an evening shift (3:00 PM – 11:00 PM) at a Connecticut senior living and healthcare facility. The role focuses on clinical intake, admissions assessments, and medication reconciliation to ensure a safe transition for new residents.

Qualifications

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC or sub-acute rehab preferred.
• Admissions or discharge coordination or desk nursing experience desirable.
• Hands-on EMR experience (e.g., PointClickCare).
• Excellent organizational, prioritization, and interpersonal skills.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM).
• Perform initial nursing assessments and admission documentation on arrival.
• Reconcile hospital discharge summaries, orders, and pharmacy profiles for continuity.
• Coordinate with discharge planners, care teams, and families for room setup and equipment.
• Initiate preliminary care plans and communicate resident needs to on‑duty supervisors.
• Orient new residents and families to policies and care routines to ease transitions.
