Qualifications

• Current CPhT Certification required and in good standing
• 2 years pharmacy experience preferred
• High School Diploma required
• Healthcare experience preferred

Responsibilities

• Review referral contents for completeness and accuracy
• Create a patient profile from scanned or faxed information
• Conduct initial prescription check for clinical accuracy
• Communicate with providers regarding referrals and questions
• Update progress notes to aid workflow
• Enter provider information (NPI) accurately
• Perform other duties as assigned
