Responsibilities

• Assesses physiologic health status and develops an individualized plan of care.
• Collaborates with physicians and health team members in coordinating treatments.
• Delivers nursing care, administers medications, and monitors patient response.
• Documents care plans, treatments, and patient progress accurately.
• Participates in discharge planning and patient/family education.
• Provides leadership and direction to care team as needed.
