Job summary

Northwell Health in New York is seeking an experienced registered nurse to provide age-specific care using the nursing process. The role emphasizes assessment, planning, implementation, and evaluation, with strong collaboration with physicians and the health team to coordinate and direct care on the unit.

The nurse will assess physiologic and psychosocial health, identify outcomes, develop and execute care plans, implement treatments, and educate patients and families.

Qualifications

• Graduate from an accredited School of Nursing; ADN required, BSN preferred.
• 1–2 years of professional nursing experience in a related specialty.
• Current RN license in NY and/or CT; BLS required.

Responsibilities

• Assess patient’s physiologic health status.
• Assess patient’s psychosocial health status.
• Identify patient outcomes and establish individualized goals.
• Formulate the plan of care and establish priorities to achieve outcomes.
• Implement the plan of care and oversee delivery of care.
• Evaluate care and measure patient outcomes.
• Collaborate with physicians and care team; provide direction to staff.
• Perform grade I-IV Decubitus care and skin care procedures.
• Administer medications safely and accurately.
• Provide patient and family education and discharge planning.
• Perform related duties as required.
