Job summary

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Professional Nurse in New York State.
• BSN degree preferred.
• 2 years of Med/Surg or Telemetry experience highly preferred.

Responsibilities

• Assesses the patient's physiologic health status and documents the plan of care.
• Assesses psychosocial health status and identifies care needs.
• Develops patient outcome statements and goals for care.
• Formulates and prioritizes the plan of care and coordinates resources.
• Implements the plan of care and monitors patient safety and responses.
• Collaborates with physicians and health team members and assists in discharge planning.
• Performs ongoing evaluation of care effectiveness and documents outcomes.
