Job summary

Qualifications

• Graduate from an accredited School of Nursing and become licensed in NY.
• Demonstrates leadership and clinical judgment in coordinating patient care.
• Excellent collaboration with physicians and health team.

Responsibilities

• Assesses physiologic health status and documents plan of care.
• Assesses psychosocial health status and identifies patient needs.
• Identifies patient outcomes and creates individualized goals.
• Formulates and prioritizes the plan of care and coordinates resources.
• Implements and reevaluates the plan of care, ensuring safety and accuracy.
• Collaborates with care team and physicians to adjust treatment as needed.
• Administers medications and provides patient education and discharge planning.
• Performs related duties and maintains thorough documentation.
• Maintains a calm environment and effective communication with family/caregivers.
