Job summary

Nestmed is seeking an Implementation Specialist to lead enterprise rollouts end-to-end across multi-branch home health and hospice accounts, designing rollout strategy and sequencing. You will own the plan, manage executive relationships, coordinate with IT, clinical leadership, and operations, and drive adoption with targeted training and change management.

This role requires 7+ years in implementation, willingness to travel up to 20%, and the ability to translate complex clinical workflows

• Strong program/project management and timeline ownership.
• Executive presence with senior leadership.
• Willing to travel up to 20% in the US.

Responsibilities

• Own end-to-end enterprise rollout plans from kickoff to full adoption.
• Design phased go-live sequencing by region or service line.
• Coordinate master implementation timeline across IT and clinical leadership.
• Lead virtual and on-site training for clinicians and staff.
• Maintain executive relationships and track health metrics.
• Manage adoption and post-go-live reinforcement.
