Job summary

Habitat Health is seeking an RN Care Manager in California to deliver personalized nursing care for a panel of PACE participants in both center and home settings. You will perform longitudinal care coordination, participate in IDT meetings, and partner with medical providers to implement care plans.

The role requires an unencumbered California RN license, 2–4 years of clinical experience with medically complex older adults, and 1–2 years in case management.

Qualifications

• Graduate of an accredited school of nursing or equivalent experience.
• Unencumbered California RN license required.
• Minimum 2–4 years of clinical experience caring for medically complex or older adults as an RN.
• Minimum 1–2 years in case management.
• Strong clinical acumen in chronic disease management and geriatric care.
• Experience in wound care, IV therapy, phlebotomy, colostomy/ileostomy care.
• CPR/BLS certification current or to be obtained within 30 days.
• Minimum one year experience working with the frail or elderly.

Responsibilities

• Conduct face-to-face nursing assessments across physical, psychosocial, and behavioral domains, in center and home settings.
• Coordinate care plans with medical providers and participate in IDT meetings.
• Deliver personalized nursing care for a panel of participants per care plans.
• Provide longitudinal case management and manage transitions of care.
• Coordinate medications, equipment, and referrals; document interventions.
• Triage in outpatient settings and initiate therapies within scope of practice.
• Educate participants and caregivers on personalized care plans.
• Assist with wound care, IV therapy, and related procedures as needed.
