Qualifications

• Proficient in documenting in electronic health records (EHR) and enrollment forms.

Responsibilities

• Conduct initial consultations to assess client needs and determine eligibility.
• Gather demographic, medical history, and insurance information for enrollment.
• Complete intake by demographic screening and data entry into EHR.
• Handle front office tasks: appointments, insurance verifications, and fee collection.
• Assist with state-funded benefits applications and community referrals.
• Coordinate with agencies and reengage clients as needed.
