Job summary

• Three years of social service, behavioral health, or complex care navigation experience using an EHR.

Responsibilities

• Help clients access services by identifying needs and routing to the proper service line.
• Maintain client engagement throughout the intake process.
• Triages clients experiencing a behavioral health crisis when needed.
• Coordinate next steps with clients and providers for referrals.
• Ensure accurate records and timely follow-up.
