Job summary

Jewish Family and Children\'s Service Inc. in Mesa, AZ seeks an Intake Specialist to support the Customer Service Referral Center.

You will handle incoming calls, verify eligibility, and schedule initial intake appointments for behavioral health and medical services. Responsibilities include confirming service availability and funding, creating referral documents in NextGen EMR, and coordinating referrals across sites.

Qualifications

• High School diploma or GED required.
• Minimum 18 years of age.
• Bilingual in Spanish & English strongly preferred (pay differential).
• Eligibility & Enrollment experience preferred.
• Experience with DCS preferred.
• Experience with NextGen EMR preferred.

Responsibilities

• Receives incoming calls and directs inquiries to appropriate resources.
• Determines whether requested services are available.
• Determines funding source availability for services.
• Schedules client appointments and verifies funding data.
• Creates Referral Demographic documents in the EHR system.
• Scans and emails referral information to relevant sites.
• Remains aware of intake capacity limits and assists clients with external resources.
• Performs other related tasks as assigned.
• Schedules transportation when necessary.
• Conducts reminder calls for appointments.

Intake Specialist - Customer Service Referral Center: The CSRC Intake Specialist receives calls from people seeking behavioral health and/or medical services, verifies eligibility for the services, and schedules initial intake appointments.

Duties include:

• Receives incoming calls
• Determines whether requested services are available.
• Determines availability of funding source to support requested services.
• Schedules client appointments, verifies funding source data, and obtains required authorizations.
• Creates Referral Demographic documents in EHR system.
• Scans/Emails copies of faxed referrals and client information to the various sites' receiving parties.
• Remains aware of agency Intake capacity limits and assists clients with accessing appropriate outside resources during those times when intake services may be closed or limited.
• Performs other related job tasks or responsibilities as assigned.
• Schedules Transportation was necessary.
• Conducts reminder calls for appointments as needed.

Qualifications:
