Job summary

Evolve-Therapy-Services is seeking a Physical Therapy Assistant to join an in-house Skilled Nursing Facility. Under the Physical Therapist's supervision, you will deliver therapy to residents, perform screenings, and help develop and implement treatment plans.

The role requires an active PTA license in the applicable states, ongoing professional development, and proficiency in documenting in NetHealth EMR per CMS requirements.

Qualifications

• Associate's degree in physical therapy from an accredited program is required.
• Active PTA license in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and in identifying goals and treatment plans with the Physical Therapist.
• Prepare and modify treatment plans and schedules with the Physical Therapist.
• Deliver individual and group treatments including strength, mobility, gait, balance, and ROM activities.
• Train residents, families, and staff in equipment use and safety.
• Document compliant, medically necessary notes in NetHealth EMR per CMS expectations.
